Specialty Nurse Practitioner

Role and Facility Information

The Ministry of Health has recently created Nurse Practitioner (NP) funded positions for NPs interested in starting patient panels within Patient Medical Homes (PMH). This Vancouver clinic is looking to include a Nurse Practitioner to join their clinical team. As part of a multi-disciplinary approach to full service family medicine, an interested NP would work collaboratively in the team environment, and start their own patient panel as per Ministry requirements.

This multi-disciplinary site has 50+ exam rooms with capacity of 25+ GP’s, NP’s and specialists. On-site pharmacy, lab, diagnostic imaging and nurses are a part of the center. There is an urgent primary care centre located on the bottom floor as well. This clinic is currently looking to recruit experienced Nurse Practitioners. The clinic runs on Medaccess EMR. This clinic is actively recruiting Nurse Practitioners and is happy to discuss the opportunity with NP’s who are interested.

• The NP can expect to work full-time with a minimum of 1,680 hours annually over 220 days.

• NP will have the requirement to build a patient panel of at least 500 in year one, 800 in year two and 1,000 is year three

• The contract term is three years

Be part of a cohesive team where each provider has their own unique role and scope in the provision of patient care and you work together to make sure that the patient’s needs are met in the most effective way possible. Responsibilities would include:

• The responsibility and accountability for the comprehensive assessment of patients/clients/residents including diagnosing diseases, disorders, and conditions is key to this role.

• The ability to Initiate treatment including health care management, therapeutic interventions and prescribe medications in accordance with the statutory and regulatory standards, limits, and conditions, and employer policies and procedures.

• The opportunity to practice autonomously and interdependently within the context of an interdisciplinary health care team while providing professional mentorship to other health professionals

• The position collaborates with patients/clients/residents and other health providers to identify and assess trends and patterns that have implications for patients/clients/residents, families and communities; develops and implements population and evidence based strategies to improve health and participates in policy-making activities that influence health services and practices.

• The position participates in peer review and self-review to evaluate the outcome of services at the patient/client/resident, community and population level.

Are you motivated to join us? We will be looking for you to have:

• Current provisional/practicing registration as a Nurse Practitioner with the BC College of Nursing Professionals (BCCNP)

• Relevant, clinical nursing experience
